Facility Rental Agreement and Pricing Guide

Before renting an SSD facility, please read, sign the following documents:

Hold Harmless Agreement
(This can be emailed or faxed to Sheila Stamschror at (509) 836-8435.)
Youth Participation Form (Must be filled out for AAU, club volleyball, or other youth team sport activities. This can be emailed or faxed to Sheila Stamschror at (509) 836-8435.)
Concussion and Cardiac Arrest Form
Concussion and Cardiac Arrest Form (Spanish) (Must be filled out and turned in with application to rent facilities.)
